Kaiser Permanente Washington is seeking a Visiting Nurse - Hospice to join our fantastic Home Care and Hospice team! Work/life balance and great leadership exists here at Kaiser Permanente!

This is a 0.8 FTE \"day\" shift position! Shifts are generally 8 hours in length, generally working 8am - 5pm Monday - Friday, with some weekend rotation among the greater team.

This position is a part of our Central branch team based out of our Capitol Hill location. An appropriate segment of this greater geographical territory would be assigned to you in this role. A team mindset is needed as this position is a part of a greater and fantastic interdisciplinary team!

The Visiting Nurse Hospice will:
  • Part of an inter-disciplinary team, which may include RNs, Medical Social Workers, Spiritual Hospice Counselors, Physical or Occupational Therapists, Speech Pathologists, MDs, Home Health Aids and a caring Clinical Team Manager!
  • Provides comprehensive skilled nursing care and services to patients in the home setting
  • Works on an interdisciplinary team to provide care in the home health, palliative care and hospice programs
  • Accountable for the assessment, development of plan of care, education and consultation related to pain and symptom management as well as adjustment to changes in health status
  • Plans for the return of patient care to primary healthcare providers as indicated


Bachelor's Degree in Nursing or other applicable healthcare related field

Preferred: Bachelors of Science in Nursing

Minimum: 2 years of experience as a Registered Nurse. Prior experience in Medical Surgical or some form of Home Health or Hospice experience
Preferred: 2 years of Home Health or Hospice experience

Knowledge/Skills Requirements
Minimum: Intermediate ability to work independently. Ability to transfer / position patients as needs require. Basic laptop skills including MS Office Suite. Customer service skills. Demonstrated experience providing care planning within a community setting (continuum of care).
Preferred: Basic or limited McKesson for homecare system experience.

Minimum: At time of hire Current Washington State RN License. Washington State driver's license with proof of insurance.
