Job DescriptionITLE: RN Care ManagerAREA OF ASSIGNMENT: Fairwood Primary Care ClinicHOURS OF WORK: DaysRESPONSIBLE TO: Clinic ManagerPREREQUISITES:Current Washington State Registered Nurse license.BSN preferredCurrent American Heart Association BLS/CPR for Health Care Providers certificationTwo years of recent clinical experience as an RN working in a clinic/outpatient setting preferred.Demonstrated knowledge and understanding of information technology.Completion of the American Association of Ambulatory Care Nurses (AAACN) Care Coordination and Transition Management (CCTM) Course within three months of hire. CCTM certification is to be completed within one year of hire. GENERAL QUALIFICATIONS:Ability to communicate effectively, in oral and written form, in the English language.
