Interviewing immediately. Apply today. As a Nurse Practitioner you will: Performs an initial comprehensive assessment on all newly enrolled patients in their home reviewing all their needs
Prescribes appropriate diagnostics and interventions to avoid unnecessary transfers/acute admissionsEducates members and/or their legal representatives in disease processes affecting members and ways to manage them effectively, as well as to promote wellness
Establishes and maintains effective relationships with patients and their legal representatives, facility staff, PCPs and other collaborating physicians/providers
Manage patient care on a longitudinal basis, with the primary objective of delivering proactive chronic disease care and significantly reducing avoidable hospitalizationsDiagnose, prescribe and treat within the scope of the Nurse Practitioner / Physician Assistant and consistent with standard of carePartner with the patients primary care provider and specialists to deliver integrated careParticipate in a rotated on-call program for home-based patient support after-hours Completion of Nurse Practitioner (NP) / Physician Assistant (PA) accredited program.2+ years of clinical experience with home based care desired Knowledge of clinical standards of careKnowledge of CPT, ICD-9 and HCPC codes.Awareness of UM standards, NCQA requirements, CMS guidelines, Milliman guidelines, and Medicaid/Medicare contracts and benefit systems is helpful.Current unrestricted Nurse Practitioner state license, and current state drivers licensePassion for geriatric care and patients
