Kaiser Permanente Washington is seeking a per diem Case Management Liaison Nurse (RN) to join our fantastic Nursing Home Services Team in Seattle, WA!

The Liaison Nurse will:
  • Focus on achievement of optimal patient health care outcomes while ensuring appropriate utilization of health care resources.
  • Working closely with Kaiser Permanente Washington teams and network providers
  • Will establish a collaborative plan of care to assure adherence to the medical plan, improvement in functional status, and improved ability to self-manage.
  • Serves as the liaison across the internal Kaiser Permanente Washington care continuum in regard to Nursing Home Services, between KPWA and all externally contracted providers, facilities, and resources and provides feedback to the organization regarding the service and quality of contracted services.
  • Individual in this role is monitoring length of stay, patient appropriateness, working with the team in the nursing home and KP physicians/practitioners social workers to make appropriate decisions.
  • The Liaison Nurse collects data and provides input to leadership regarding issues or concerns related to utilization, cost, quality, service and care delivery to patients

* Travel includes a once per week visit to the contracted facility, which may be done via phone depending on the needs of the business. If travel is involved to a local facility, travel is anticipated to be no more than 10%.


Minimum Education Requirements
Bachelors Degree BA or BS.
Preferred Education
Bachelors Degree BS in Nursing.

Minimum Knowledge/Skills Requirements
Intermediate Effective, independent nursing judgment and skills, and use of evidence based clinical decision making criteria.
Advanced Knowledge in management of chronic disease process, nursing process and collaborative care planning. Demonstrated skill and experience in effectively collaborating with care team members.
Preferred Knowledge/Skills
Telephonic Case Management, acute or payor-based Utilization Review

Minimum Experience Requirements
3 years recent RN clinical experience in medical/surgical or ambulatory clinical, nursing home or similar experience.

2 years' RN experience in ambulatory case management, care coordination or disease management, or similar
Preferred Experience
Experience with health plans, telephonic case management, utilization review, hospitals, disease management.

Minimum License/Certifications Requirements
At time of hire Current Washington State RN license.
At time of hire Valid WA State driver's license if in a Traveler role.
Able to obtain shortly after hire Certification in case management field within two years of hire.
Able to obtain shortly after hire CPR within 30 days of hire.
