We are hiring for a Liaison Nurse at Providence Everett Hospital to focus on achievement of optimal patient health care outcomes while ensuring appropriate utilization of health care resources.

This position will work variable days, every other weekend, Rotating Holidays at Providence Everett Hospital. Days and hours are variable between 8:00AM - 4:30PM

In this position you will:
  • Work closely with primary care teams, specialty care teams and medical providers.
  • Work directly with patients and patients families (patient facing vs telephonic role)
  • Establish a collaborative plan of care to assure adherence to the medical plan, improvement in functional status, and improved ability to self- manage.
  • Serve as the liaison across the internal care continuum and all externally contracted providers, facilities, and resources and provides feedback to the organization regarding the service and quality of contracted services.
  • Collect data and provides input to leadership regarding issues or concerns related to utilization, cost, quality, and service and care delivery to patients.



Minimum Education Requirements

Bachelor's Degree (BA or BS) in addition to Associates Nursing Degree from an accredited program

Preferred Education

Bachelors Degree BS in Nursing.


Minimum Knowledge/Skills Requirements
  • Effective, independent nursing judgment and skills, and use of evidence based clinical decision making criteria.
  • Advanced Knowledge in management of chronic disease process, nursing process and collaborative care planning.
  • Demonstrated skill and experience in effectively collaborating with care team members.


Minimum Experience Requirements

3 years Recent RN medical/surgical/ambulatory clinical experience required.

Preferred Experience:

2 years RN experience in ambulatory case management, care coordination or disease management in addition to minimum experience requirements.


Minimum License/Certifications Requirements
  • At time of hire Current Washington State RN license.
  • Able to obtain shortly after hire Certification in case management field within two years of hire.
  • Able to obtain shortly after hire CPR within 6 months of hire.
