Some travel involved to meet with clinical care teams for patient and family conferences.

Primary responsibility is to focus on achievement of optimal patient health care outcomes while ensuring appropriate utilization of health care resources. Working closely with primary care teams, specialty care teams and medical providers, the Liaison Nurse will establish a collaborative plan of care to assure adherence to the medical plan, improvement in functional status, and improved ability to self- manage.
This is a patient facing role working with patients and patients families vs telephonic role.

Serves as the liaison across the internal care continuum and all externally contracted providers, facilities, and resources and provides feedback to the organization regarding the service and quality of contracted services. The Liaison Nurse collects data and provides input to leadership regarding issues or concerns related to utilization, cost, quality, and service and care delivery to patients.

This is a .9 FTE or 36 hours per week - working variable days and every other weekend at Swedish Hospital, both First Hill and Cherry Hill, with possible float to Providence Everett Hospital.


Minimum Education Requirements
Bachelor's Degree (BA or BS) in addition to Associates Nursing Degree from an accredited program
Preferred Education
Bachelors Degree BS in Nursing.

Minimum Knowledge/Skills Requirements
Effective, independent nursing judgment and skills, and use of evidence based clinical decision making criteria.
Advanced Knowledge in management of chronic disease process, nursing process and collaborative care planning. Demonstrated skill and experience in effectively collaborating with care team members.

Minimum Experience Requirements
3 years Recent RN medical/surgical/ambulatory clinical experience required.
Preferred Experience:

2 years RN experience in ambulatory case management, care coordination or disease management in addition to minimum experience requirements.

Minimum License/Certifications Requirements
At time of hire Current Washington State RN license.
Able to obtain shortly after hire Certification in case management field within two years of hire.
Able to obtain shortly after hire CPR within 6 months of hire.
