Summary This position provides outreach education and low level case management in support of youth being served by the School Services program This includes developing and or implementing site specific outreach strategies designed to increase awareness of and utilization of services at our school partner sites This may include working with the Mobile Dental Clinic or School Based Health Services as necessary This position recruits participants for education and support services and links students and families to other community resources Duties and Responsibilities Provide outreach and information about school health services available at the school based health center or mobile dental clinic to students and families Recruit new patients students and follow up with existing patients students as needed Assess student and family needs and provide information or referrals to social service or other community resources as needed follow agency protocols for referral follow up and quality Collaborate with health providers and clinic coordinators to respond to individual student and family needs related to wellness and school success Participate on student health centered teams and work with students and families individually or in groups to gather information surveys focus groups etc to help inform services Provide insight and consultation to project partners on developing program options that are culturally responsive to the target populations Provide assistance in preparing and delivering health education groups and prevention education workshops for students and families Maintain communication with and between team members school partners students and families to help ensure program success Assist team members with ongoing continuous quality improvement to enhance program operations including problem solving helping to develop program systems and procedures or participating on team workgroups Maintain timely accurate and complete records of all client activities and outreach education activities in line with agency and funder requirements Provide outreach and information on ICHS clinics and services to community members and partner organizations as appropriate Develop and utilize a referral network with other service agencies for referrals to help support needs of clients Represent school services programs at school and community events Evenings and weekends may be required Participates in the development and implementation of goals work plans performance measures and continuous improvement of service delivery to attain program department and organization goals Complies with all applicable federal state and local laws regulations and standards and ICHS policies and procedures Adheres to organizationa s Safety and Health and Accident Prevention programs and actively supports and participates in the Emergency Management program and activities Participates in the development of special projects agency events and health fairs in consultation with supervisor Case management for insurance eligibility enrollment May provide interpretation and translation as able Job Requirements Education a Associate Degree in health related field plus year experience providing outreach in health care or middle high school setting Experience a Minimum of year of experience in a health care or school setting community health education and or outreach position Experience working with schools parents and middle and high school aged youth preferred Valid Washington Drivera s License insurance and access to a vehicle Must pass ICHS background check requirements additional background checks may be required for work in schools Bilingual English Spanish preferred
